Workplace Injury Reporting

Please do what you can to make your work environment and your lab space, safe for you and your fellow employees and students.

If you are injured at the workplace, you must report the injury to your supervisor AND report the injury within 24 hours, by using the form at the following website.

Supervisors are responsible for enforcing this policy.

Duke, of course, is interested in minimizing costs and law suits, and wants to be sure that any medical treatment that is required is done immediately so costs don't escalate because of complications due to waiting.

Please be aware that if you do not report an injury within 24 hours, the Physics Department will be liable for the cost of your medical care. Thus, it is in our best interest that this doesn't happen.
